2017-18 Health Requirements

Dear Parent/Guardian:

Summer is passing by quickly!  We wanted to send out one final reminder of what is needed by the school nurse before classes begin on August 24th:

1. All returning students should complete a “returning student health history” form.   Please complete this form even if you register online.

2. All 7th graders are required to have both a meningococcal vaccine and a Tdap (tetanus, diphtheria, and pertussis) booster before school starts this fall.

3. All 9th graders should have a dental screening performed.  This is an Iowa requirement for all students entering kindergarten and 9th grade.  If your child recently had an exam (within the past 6 months) you can take this form into your dentist or hygienist to sign.

4. All 12th graders are required to have a meningococcal vaccine before school starts this fall.

5. All students with severe allergies and/or asthma should have their health care provider complete an “Allergy Anaphylaxis Plan” and/or an “Asthma Action Plan”.

Many of you have already completed and returned these forms to us.  We graciously thank you for your promptness.  If you have misplaced your forms(s), you can find them on the Gilbert website under the “Parent/Student” tab.  If you have any questions about the new immunization requirements (meningococcal), please read the information found on the Iowa Department of Public Health website at www.idph.iowa.gov.  Click on the section titled “Iowa Immunization Requirements”.  Your healthcare provider can also answer questions regarding the vaccine.

Laura Metzger, RN will be returning to Gilbert next fall for the 2017-18 school year.  Again, we had a great year working with the Gilbert students!  Enjoy the rest of your summer!
